We are pleased to announce patient portal services through our EHR system - ICANotes.  Our office will send a welcome link to the portal once you provide your personal email.

Please register with a username and password and follow these steps:

​

1. click on "profile" located at the top of the page, provide current information

2. click on "patient history" and complete. 

 

These sections are helpful to your clinician and will be reviewed prior to your appointment if done in a timely manner.  The patient portal allows access to future appointments that can be confirmed, canceled or rescheduled.  You are also able to send messages and upload important information for staff and/or  clinicians.
